What should I expect on my first visit?
We are located in Clinic C in the Huntsman Cancer Institute. Please arrive a few minutes early to fill out new patient paper work which includes an information sheet and a brief medical/surgical history.
A medical assistant will bring you back to an exam room, check your vital signs, and review your medications, so please bring a list with dosages.
The physicians will examine you and discuss tests and treatment options with you. Occasionally the doctors run behind schedule because they like to spend quality time with each patient to answer all questions. You may want to bring a book in case you have to wait. HCI has wireless Internet access that you may use if your laptop computer is equipped for it.
What should I bring?
Please bring the following materials to your appointment:
- Appointment schedule or calendar
- Your medical and dental insurance cards
- Referral or authorization documents
- Co-payment (if applicable)
- Phone numbers where you can be contacted
- List of questions you may have for your doctor
- Other applicable medical information you might have, such as:
- Test results
- Lab reports
- Medical history charts
- Diagnostic films
- CT or MRI scans (the actual films or disc)
- A list of your medications along with their dosages
- A list of your medical allergies and your type of reaction

How Should I Dress?
Casual clothing and comfortable shoes are appropriate. The average summer temperature in Salt Lake City is 92.9 degrees Fahrenheit, while the average winter temperature is 36.4 degrees Fahrenheit. It is a good idea to bring a sweater because room temperatures can be cool any time of year.
What number do I call to find out what time my surgery is scheduled?
You must call after 2 p.m. the day before your surgery to find out the time the operating rooms have scheduled you.
If your surgery will be at HUNTSMAN CANCER HOSPITAL, call 801-587-4300

How do I get copies of my medical records?
You must submit a request in writing, which you can mail to the address below or hand-deliver to Clinic 9 at the University Hospital. Be sure to include the name of the physcian(s) you see.
